Job Responsibilities

• Maintain accurate and timely documentation of patient progress and treatment plans.

Qualifications

• Current and valid Physical Therapist license in the state of Wisconsin.
• Previous experience in Home Health is preferred but not required.
• Strong interpersonal and communication skills.
• Ability to work independently and manage a flexible schedule.
• CPR certification is required.
